Change the surname in my SBI Account.?

Before Some days ago I to join my wife name with my SBI Account. but SBI do some mistake in SURNAME. SBI Put her Surname form her father's. so now I want to do here surname in SBI A/c. they want required application for change the surname. so pls. give me application letter format.
